Echocardiographic Patterns of Mitral Valve Systolic Anterior Motion in Obstructive Hypertrophic Cardiomyopathy

چکیده
Background. The morphological determinants of mitral valve systolic anterior motion (SAM) and obstruction to left ventricular outflow in patients within the broad clinical spectrum of hypertrophic cardiomyopathy (HCM) are not completely understood, particularl the contribution of mitral leaflet length and size. Methods and Results. To clarify this issue, mitral valve specimens from 43 patients with HCM and basal outflow obstruction were used to relate morphometric measurements of leaflet area to certain morphological and functional assessments of left ventricular outflow tract geometry and valvular motion obtained from echocardiograms in the same patients. Twenty-four patients (56%) had mitral valves of normal size (leaflet area < 12.0 cm2) and 19 patients (44%) had enlarged and elongated valves (area >12.0 cm2). Compared with normal-sized mitral valves, the enlarged valves were situated more posteriorly in a larger left ventricular outflow tract (cross-sectional area, 3.3±1.0 versus 1.9±0.7 cm2 for normal-sized valves; p<0.001) and also had greater systolic excursion of the anterior leaflet

Echocardiographic mitral systolic motion in left ventricular aneurysm.
An abnormal, convex systolic anterior motion of the mitral valve, somewhat similar to that seen in idiopathic hypertrophic subaortic stenosis (hypertrophic obstructive cardiomyopathy) and occasionally in atrial septal defect, was found on the echocardiogram in 8 out of 9 patients with angiographically (7 patients) and/or clinically (2 patients) diagnosed left ventricular aneurysm secondary to m...
متن کاملHypertrophic obstructive cardiomyopathy. Assessment by echocardiographic and Doppler ultrasound techniques.
Hypertrophic obstructive cardiomyopathy is a disease of the myocardium that can be assessed echocardiographic and transcutaneous Doppler ultrasound techniques. Four patients are presented with various patterns of the disease, and the frequently familial incidence is illustrated. The importance of ultrasonic evidence for asymmetric septal hypertrophy in all stages is emphasized and evidence of r...
متن کاملMechanism of systolic anterior motion of mitral valve and site of intraventricular pressure gradient in hypertrophic obstructive cardiomyopathy.
The mechanism of systolic anterior motion of the mitral valve and the localisation of the intraventricular pressure gradient were determined in 15 cases of hypertrophic obstructive cardiomyopathy by the combined use of real time two dimensional echocardiography and intracardiac manometry. We arrived at the following conclusions. The systolic anterior motion of the mitral echo in the M-mode echo...
متن کاملThe mitral valve in hypertrophic cardiomyopathy--an echocardiographic approach.
In the recent past, the mitral valve was referred to as the cornerstone of any echocardiographic examination. Changes in its echocardiographic pattern were considered, in many cases, to be specific for the diseases in which they were described. Among such diseases was hypertrophic cardiomyopathy. Three abnormalities were successively described in the M-mode echocardiograms of patients with the ...
متن کاملEchocardiographic differentiation of hypertensive heart disease and hypertrophic cardiomyopathy.
The clinical differentiation of hypertensive heart disease from hypertrophic cardiomyopathy usually presents no problem but it is less clear whether an echocardiographic distinction can always be made and, if so, what those echocardiographic criteria of difference are. It can be inferred from recent publications that when echocardiographic criteria for hypertrophic cardiomyopathy are met in hyp...
